STRUCTURED CARE PACKAGE

 

A structured care package is based around what tasks a service user would need assistance or help with.  These tasks are then normally split into four parts of the day - AM Call, Lunch Call, Tea Call and PM Call.  These calls can be provided from 15, 30, 45 and 60 minutes at a time and will be assessed to have either one or two Social Care workers to carryout the tasks.

AM Call

AM calls is the part of the day where all elements of personal care are provided. This can be:

  • Assistance with toileting
  • Full body wash
  • Showering
  • Shaving
  • Application of prescribed creams
  • Dressing
  • Transferring to seating arrangment
  • Preparing breakfast (Assist with feeding if required)
  • Administering prescribed medication from a sealed unit E.g. Blister pack or Pill Pac Plus

Lunch Time Call

Lunch Time Calls are mainly for any toileting needs, meal preparation (assistance with feeding if required) and medication administering.  Depending on the allocated time any other needs that require s Social Care Worker(s) assistance can be completed.

Tea Time Call

Tea Time Calls are mainly again for toileting needs, meal preparation (assistance with feeding if required) and medication administering.  Our Social Care Workers can assist with any other needs if the allocated time allows them.

PM Call

PM Calls are to provide assistance with transferring into to bed or providing assistance with bed preparation.  Task such as:

  • Undressing / dressing for bed
  • Toileting and hygiene
  • Incontinence product management
  • Personal Care (if required)
  • Preparation of supper
  • Medication administration

At the beginning of any Care Package a member of The Essential Homecare Services Management will contact you to arrange an introduction meeting.  During this meeting we will go through the discussed tasks and times of service requirements, this allows us to assess the service user and ensure all tasks can be carried out safely.  From this initial introduction meeting a Timetable of Services (TTS) will be drawn up.  This document will display

  • Service User Name
  • Address
  • Date of Birth
  • Contact Number
  • Next of Kin Details
  • Health Care Number
  • Doctor Name and Surgery Address
  • Amount of calls during the day
  • The Length and Times of The Calls
  • The Tasks to Be Carried Out
